Frequently Asked Questions

We are not in network with any insurance companies, and payment is due in full at the time of service.  We provide a Superbill (a detailed receipt) that can be submitted to insurance for reimbursement, and are happy to provide assistance with that process if needed.  Here are some links to reimbursement forms/procedures for some of the more common insurance plans:

Missed appointments and appointments cancelled with less than 24 hours’ notice may be subject to the full appointment fee. If you or your child develop an acute illness or new symptoms that prevent in-person attendance, please notify us as soon as possible.  The cancellation fee may be waived in these circumstances.

In the event of inclement weather, please contact us at 503-444-1745 to confirm your appointment status. We will make every effort to notify you by phone and/or email in advance if the office is closed due to weather-related concerns. If weather conditions make it unsafe for you to attend your appointment in person, we can convert your appointment to a virtual visit or reschedule as needed. No cancellation fee will be charged for rescheduling in-person appointments due to inclement weather.
